



Articular cartilage in joints, which is not part of the blood circulation system, receives nutrients through the movement of the joint, which promotes the nourishing flow of synovial fluid.
Cartilage relies on the diffusion of nutrients from synovial fluid, which is stimulated by the movement of the joint. The compression and release of the cartilage during movement help circulate synovial fluid, which carries the necessary nutrients to maintain cartilage health. This process is critical for cartilage because, without a direct blood supply, it depends on joint activity to support its nutrition and repair processes.
Cartilage is made of water, collagen, and proteoglycans, a protein-polysaccharide bond that provides elasticity.
As tissues go, cartilage is fairly isolated: it doesn’t have a direct blood supply, so it cannot receive blood-borne nutrients in the same way as most tissues. Instead, it depends on the surrounding synovial fluid for nourishment and maintenance, which is facilitated by joint movement.
Cartilage also lacks nerve endings, which means it does not generate pain or other sensory signals directly. This is why damage to cartilage, such as in osteoarthritis, may progress without immediate pain until other structures, like the surrounding bones or tissues with nerves, are affected.
So while cartilage is essential for smooth joint function, its isolation can make healing and detecting early damage more challenging.
Intervertebral discs, the cartilage structures between the vertebrae of the spine, do not receive nourishment from synovial fluid like the cartilage in joints. Instead, these discs rely on diffusion for nutrient exchange, as they lack their own direct blood supply. The outer layer of the disc, called the annulus fibrosus, has some connection to blood vessels in the surrounding tissues, while the inner, gel-like nucleus pulposus is nourished through diffusion from nearby vertebral endplates, which are thin layers of cartilage that cover the adjacent vertebrae. Movement and compression of the spine during daily activity help facilitate the diffusion of nutrients into the disc and the removal of waste products, making regular, low-impact motion essential for maintaining disc health.
