
The glymphatic system is the brain’s waste-clearance system. Although it has been studied most extensively in humans and laboratory animals, current evidence suggests that horses possess a similar system that likely functions in much the same way. Equine-specific research is still emerging, but our understanding continues to grow.
The glymphatic system helps remove metabolic waste products—including proteins, inflammatory molecules, and other byproducts of normal brain activity—from the central nervous system. By clearing these substances, it helps maintain a healthy environment for nerve cells and supports normal brain function.
How the Glymphatic System Works
Unlike most tissues in the body, the brain does not contain conventional lymphatic vessels within its tissue.
Instead, cerebrospinal fluid (CSF) travels alongside arteries into the brain, where it exchanges with the fluid surrounding brain cells. As this fluid moves through the brain, it collects metabolic waste before exiting alongside veins.
This process is facilitated by specialized water channels called aquaporin-4, which are located on supporting brain cells known as astrocytes.
The waste-containing fluid then enters the meningeal lymphatic vessels, a recently recognized network that carries it toward the deep cervical lymph nodes, where it rejoins the body’s lymphatic circulation.
Why Sleep Matters
The glymphatic system is most active during deep, restorative sleep.
During sleep:
- Brain cells shrink slightly, creating more space for fluid movement.
- Cerebrospinal fluid circulates more efficiently.
- Clearance of metabolic waste increases substantially.
Conversely, inadequate sleep, chronic stress, persistent pain, and certain neurological disorders may reduce the efficiency of glymphatic function.
This highlights the close relationship between brain health, nervous system regulation, and quality sleep.
Where Massage May Fit
At present, there is no direct evidence that massage therapy enhances glymphatic clearance in horses.
However, massage may influence several physiological processes that help create the conditions in which the glymphatic system functions most effectively.
Massage may:
- Encourage parasympathetic (“rest and digest”) nervous system activity while reducing excessive sympathetic (“fight or flight”) activation.
- Improve comfort, making restful sleep more likely.
- Reduce muscular and fascial tension around the poll, neck, and thoracic inlet, where important vascular and lymphatic structures pass.
- Support healthy venous and lymphatic drainage from the head and neck.
- Improve tissue mobility around blood vessels, nerves, and fascial planes.
- Encourage relaxed diaphragmatic breathing, which assists venous return and lymphatic movement throughout the body.
Rather than acting directly on the brain’s waste-clearance system, massage may help optimize the physiological environment that allows the body to carry out these processes naturally.
The Importance of the Neck
Fluid leaving the brain ultimately drains into lymphatic pathways located in the neck.
For this reason, the poll, atlanto-occipital region, upper cervical fascia, and thoracic inlet are anatomically significant. These regions contain major vascular, fascial, and lymphatic pathways through which fluid exits the head.
Although it has not yet been demonstrated that restrictions in these tissues alter glymphatic function in horses, their anatomical relationships suggest they are worthy of continued research.
For bodyworkers, this reinforces an important principle: the head and neck should not be viewed simply as isolated musculoskeletal structures. They are part of an integrated system involving the nervous system, circulation, fascia, lymphatic drainage, respiration, and movement.
An Integrated System
One of the most exciting developments in modern neuroscience is the growing appreciation that movement, fascia, circulation, breathing, sleep, lymphatic function, and the nervous system are deeply interconnected.
Rather than operating independently, these systems continually influence one another. Changes in one often affect the others.
Massage is unlikely to influence the glymphatic system through a single mechanism. Instead, its greatest contribution may be supporting whole-body regulation by reducing unnecessary muscular tension, improving tissue mobility, encouraging relaxation, promoting comfortable movement, and creating conditions that support the body’s own maintenance and recovery processes.
The Bigger Picture
The discovery of the glymphatic system has expanded our understanding of how the brain maintains its internal environment. Although much remains to be learned about this system in horses, current evidence suggests it plays an important role in supporting normal neurological function.
For equine bodyworkers, the glymphatic system provides another reminder that the body functions as an integrated whole. Healthy movement, quality sleep, efficient circulation, relaxed breathing, balanced nervous system activity, and tissue mobility all contribute to the conditions under which the body’s natural regulatory systems perform their best.
Rather than “removing toxins,” massage therapy is best understood as supporting the horse’s own remarkable ability to regulate, adapt, and maintain itself—a perspective that aligns closely with our growing understanding of equine physiology.
